1. About these Terms

These Terms & Conditions apply to physiotherapy, rehabilitation, exercise rehabilitation, scar therapy, virtual physiotherapy and other services provided by Rajvi Shah under the trading name Physiorehab by Rajvi Shah (“Physiorehab”, “I”, “me”, “my” or “the Practice”).

By booking or receiving services, you agree to these Terms & Conditions.

These Terms do not affect your statutory rights as a consumer.

Where there is any conflict between these Terms and mandatory consumer, healthcare, data protection or other applicable law, the applicable law will take precedence.

2. About the Practitioner

Physiorehab by Rajvi Shah is a sole trader business operated by:

Rajvi Shah

Rajvi is an HCPC registered physiotherapist.

HCPC Registration Number: PH117801

Rajvi is also a member of the Chartered Society of Physiotherapy (CSP).

Physiotherapy services are provided within the practitioner’s professional scope of practice, training, knowledge, skills and experience.

3. Services

Services may include, depending on clinical suitability:

  • Physiotherapy assessment
  • Musculoskeletal physiotherapy
  • Exercise rehabilitation
  • Physiotherapist-led strength rehabilitation
  • Oncology rehabilitation
  • Prehabilitation and rehabilitation around cancer treatment
  • Scar rehabilitation/scar therapy
  • Virtual physiotherapy where clinically appropriate
  • Education and self-management advice
  • Other physiotherapy-related services specifically described on the website

Not every service is suitable for every person.

The practitioner will use professional judgement to determine whether a particular service or intervention is appropriate.

4. Clinical Assessment

An appointment does not guarantee that a particular treatment, technique or exercise programme will be provided.

Following assessment, the practitioner may:

  • provide physiotherapy treatment;
  • provide exercise or rehabilitation advice;
  • modify the proposed treatment;
  • recommend further assessment;
  • recommend medical review;
  • recommend referral to another healthcare professional;
  • recommend that treatment is not appropriate at that time.

Clinical decisions are made according to the individual’s presentation, history, examination findings, goals, risks, treatment status and professional judgement.

5. Scope of Practice

Physiotherapy will be provided within the practitioner’s scope of practice.

If your needs fall outside the practitioner’s knowledge, skills, experience or scope of practice, you may be advised to seek assessment or treatment from another healthcare professional or service.

This may include referral to your GP, consultant, oncology team, specialist physiotherapist, surgeon, nurse, emergency service or another appropriately qualified professional.

6. Consent

Valid consent will be obtained before assessment and treatment.

You have the right to ask questions, request further information, refuse an intervention or withdraw consent.

Withdrawing consent does not prevent the practitioner from explaining potential consequences or recommending appropriate alternative care.

Where clinically relevant, consent may need to be revisited during treatment.

7. Your Responsibilities

You agree to provide information that is reasonably relevant to your care.

This may include information about:

  • symptoms
  • medical conditions
  • previous surgery
  • current treatment
  • medication
  • allergies
  • relevant investigations
  • relevant medical or cancer treatment
  • previous physiotherapy
  • changes in health status

You should tell the practitioner if your health changes or if you develop new symptoms.

You should follow agreed rehabilitation advice within your own capabilities and seek medical advice where appropriate.

You should not perform an exercise or treatment technique that causes concerning or unexpected symptoms.

8. Accuracy of Information

Safe clinical decision-making depends partly on accurate information.

If relevant information is withheld, unavailable or inaccurate, the practitioner may not be able to provide the intended service safely.

The practitioner may therefore modify, postpone or stop treatment where necessary.

9. Appointments

Appointments are provided for the duration stated at the time of booking.

Please arrive or connect on time.

If you arrive late, the appointment may need to finish at the originally scheduled time so that other patients are not disadvantaged.

Where clinically appropriate, the practitioner may use professional discretion regarding late appointments.

10. Fees

Current fees are displayed on the Pricing page or communicated before booking.

Fees may change from time to time.

The applicable price will be confirmed before a booking is completed.

Unless otherwise stated, payment is due in accordance with the booking or invoice arrangements.

11. Cancellation and Missed Appointments

The separate Cancellation Policy forms part of these Terms & Conditions.

If you need to cancel or rearrange an appointment, please provide as much notice as possible.

The Practice may charge a late cancellation or non-attendance fee where permitted by law and where the applicable terms have been communicated to you before booking.

The Practice aims to apply cancellation charges reasonably and proportionately.

A cancellation charge will not remove or restrict any statutory consumer rights that apply.

Exceptional circumstances may be considered at the practitioner’s reasonable discretion.

12. Statutory Cancellation Rights

Where consumer law gives you a statutory right to cancel an online or distance contract, nothing in these Terms is intended to remove that right.

Where you expressly request that a service begins before the end of an applicable statutory cancellation period, you may be required to pay for services already provided before cancellation, where permitted by law.

Where a service has been fully performed following your express request and acknowledgement of the consequences of early performance, any statutory cancellation right that has legally ended will be treated accordingly.

If you are unsure about your cancellation rights, please contact the Practice.

13. Virtual Physiotherapy

Virtual appointments may be provided where clinically appropriate.

The practitioner will determine whether remote assessment and management are suitable.

Some conditions require physical examination or face-to-face assessment.

A virtual appointment may therefore result in a recommendation for face-to-face assessment, medical review or referral.

You are responsible for ensuring that you have an appropriate private and safe environment for the consultation and that your device and internet connection are suitable.

The Practice cannot guarantee uninterrupted internet or technology services.

14. Oncology Rehabilitation

Oncology rehabilitation is individualised.

The appropriateness of exercise or physiotherapy may depend on factors including:

  • cancer diagnosis
  • treatment stage
  • surgery
  • chemotherapy
  • radiotherapy
  • hormonal treatment
  • immunotherapy
  • medication
  • blood results
  • symptoms
  • cardiovascular or respiratory status
  • fatigue
  • other medical conditions
  • medical advice from the treating team

Physiotherapy does not replace medical, surgical or oncology care.

Where appropriate, the practitioner may recommend communication with or referral to your healthcare team.

15. Scar Therapy

Scar therapy is only provided where clinically appropriate.

The practitioner may postpone or decline treatment where there are concerns regarding:

  • wound healing
  • infection
  • wound breakdown
  • bleeding
  • unexplained swelling
  • significant pain
  • postoperative complications
  • other medical concerns

Scar therapy does not guarantee removal, disappearance or complete resolution of a scar.

Outcomes vary between individuals.

16. Exercise and Rehabilitation

Exercise recommendations are individualised.

No exercise programme can guarantee a particular result.

You should follow the instructions provided and inform the practitioner about significant changes in symptoms, health or ability.

If an exercise produces severe, unusual, rapidly worsening or concerning symptoms, stop the exercise and seek appropriate medical advice.

17. Medical Emergencies

Physiorehab is not an emergency medical service.

If you experience a medical emergency, call 999 or attend an appropriate Emergency Department.

For urgent but non-life-threatening concerns, contact NHS 111, your GP or the appropriate healthcare professional.

Do not delay emergency medical assessment while waiting for a physiotherapy appointment.

18. Confidentiality

The Practice will treat information about you as confidential and will handle personal information in accordance with applicable data protection law and professional obligations.

Information may be disclosed where there is an appropriate lawful basis, including where:

  • you have provided permission;
  • disclosure is necessary for your care;
  • disclosure is required or permitted by law;
  • disclosure is necessary in the public interest or to protect safety;
  • another lawful basis applies.

Further information is contained in the Privacy Policy.

19. Clinical Records

Appropriate clinical records will be maintained.

Records will be kept securely and retained in accordance with applicable legal, regulatory and professional requirements.

You may have rights relating to access to your personal information. Further information is contained in the Privacy Policy.

20. Communications

Routine communications may be made using the contact details you provide.

Please do not send detailed medical histories, photographs of clinical conditions, test results or other sensitive health information through general website enquiry forms or unsecured social-media messages.

Where clinical information is required, the Practice will use an appropriate clinical communication or booking process where available.

21. Social Media

Social media is not an appropriate channel for detailed clinical assessment or emergency medical advice.

If you send a clinical question through social media, the Practice may direct you to an appropriate booking or healthcare route rather than providing personalised clinical advice through that channel.

22. Website Information

Information on the website is provided for general educational purposes.

It is not a substitute for individual clinical assessment or personalised medical advice.

Website information may not be appropriate for every person.

The Practice aims to keep clinical information accurate and evidence-informed but cannot guarantee that every page will reflect every subsequent development in medical research.

23. Testimonials and Reviews

If you provide a testimonial or review, the Practice may use it for marketing only where appropriate permission has been obtained.

The Practice will not knowingly present a testimonial as genuine if it is not genuine.

Testimonials represent individual experiences and do not guarantee that another person will achieve the same outcome.

24. Intellectual Property

Unless otherwise stated, website text, graphics, branding, photographs and other content are owned by or licensed to the Practice.

You may view and use the website for personal purposes.

You must not reproduce, redistribute, modify or commercially exploit website content without appropriate permission.

25. Website Availability

The Practice aims to keep the website available and accurate but does not guarantee uninterrupted availability.

The website may occasionally be unavailable for maintenance, technical reasons or circumstances outside the Practice’s reasonable control.

26. Third-Party Websites

The website may contain links to external websites or resources.

The Practice does not control external websites and is not responsible for their content, availability, privacy practices or security.

27. Liability

Nothing in these Terms excludes or limits liability where it would be unlawful to do so.

Nothing in these Terms excludes or limits liability for death or personal injury caused by negligence, fraud or fraudulent misrepresentation, or any other liability that cannot legally be excluded or limited.

Nothing in these Terms removes your statutory consumer rights.

Subject to the above, the Practice is not responsible for losses arising from circumstances outside its reasonable control or from inappropriate use of general website information.

28. Complaints

The Practice takes complaints seriously.

If you are unhappy with any aspect of your care or service, please contact the Practice as soon as reasonably possible.

The Complaints & Feedback Policy explains the process for raising and resolving concerns.

29. Changes to These Terms

These Terms may be updated from time to time.

The version applicable to a new booking will be made available before the booking is completed.

Changes will not retrospectively remove rights that have already accrued.

30. Governing Law

These Terms are governed by the law of England and Wales.

Nothing in these Terms prevents you from exercising any statutory consumer rights available to you.
